CMS Approval Press Release  (4/5/07) - The press release from the federal department of Health and Human Services announcing Iowa as the first state to receive approval for state plan HCBS services.

Habilitation Services Parent Cost Report  (3/16/07) - Cost reporting form to be completed in addition to the Financial and Statistical Report (Form 470-4425) for providers with multiple provider numbers to designate multiple locations.
