Program of All-Inclusive Care for the Elderly (PACE)

This program helps the member and health providers come together for preventive care, primary care, social services, therapeutic recreation,acute and long-term care services. This program aims to protect and improve your health; and your quality of life. The goal of PACE is to keep you in your home.

YOU ARE ELIGIBLE FOR PACE IF YOU:

  • Are 55 years of age or older
  • Reside in a county where PACE is provided
  • Live in a community home (at time of initial PACE approval)
  • Have Medicaid, dual Medicaid/Medicare or Medicare eligibility
  • Meet the requirements for nursing facility level of care

THE PACE CENTER

The minimum services that must be available to members include:

  • Primary medical care, including physician and nursing services
  • Social services
  • Restorative therapies, including physical therapy and occupational therapy
  • Personal care and supportive services
  • Nutritional counseling
  • Recreational therapy
  • Meals

INTERDISCIPLINARY TEAM

The team will help you plan and coordinate your care across any number of support settings that may include home, hospital, nursing facility, or hospice care.

The team must include the following professionals:

  • Primary Care Physician (PCP)
  • Registered Nurse
  • Master's level social worker
  • Physical Therapist
  • Occupational Therapist
  • Recreational Therapist
  • PACE center manager
  • Home Care Coordinator
  • Personal care attendant
  • Transportation Driver
  • Dietician
